Case Manager
Job Description
A mission-driven nonprofit organization in Santa Monica, CA, dedicated to supporting at-risk youths, is seeking a Case Manager to oversee a caseload of approximately 10 youths. This role involves intensive mobile case management and maintaining regular communication with community stakeholders, such as schools, probation officers, and various community-based organizations. As a Case Manager, you will be directly responsible for providing high-quality support to at-risk youth, helping to foster their growth, resilience, and successful reintegration into their communities. The position is primarily onsite, requiring "in the field" work, with travel up to 1-2 hours away. Salary Range: $55,000 - $65,000 / year DOE
Case Manager Responsibilities:
- Provide intensive mobile case management to approximately 10 youths
- Maintain regular communication with community stakeholders, including CBOs, school administrators, and probation officers
- Manage client case files and resources in the Apricot database
- Assist in the development and facilitation of academic workshops and other empowerment activities for youth
- Participate in networking, service provider meetings, and serve as an ambassador
- Attend relevant trainings related to case management and leadership
Case Manager Requirements:
- Master's Degree in Social Work
- At least 1 year of experience as a case manager and working in the nonprofit sector
- Experience working with at-risk youth, including foster and/or formerly incarcerated youth
- Valid Class C Driver s License, proof of insurance, and access to reliable transportation
- Proficiency in Microsoft Office Suite, Teams, and Zoom
- Lived experience with the youth demographic served is preferred
- Bilingual in English and Spanish is preferred
- Must be willing to travel up to 1-2 hours away to meet with the program participants
